Otis torments Denise using the skin of her dead father's face as a mask. Mary attempts to run away, but is tracked down and stabbed to death by Baby moments later. He is presumably the father of Mama Firefly, previously known as Gloria Teasdale. She, in turn, is the biological mother of Baby, Rufus, and Tiny, as well as the presumed adoptive mother of Otis, whose last name was previously Driftwood. Captain Spaulding and Mama are Baby's parents, making her the half-sister of Rufus and Tiny. Although a biopic of Marx that Zombie was supposed to direct has yet to come to fruition, Marx's work will forever live on through Zombie's characters.

As she wanders through tunnels filled with mutilated corpses, she encounters Dr. Satan and a number of mental patients; Jerry is on Dr. Satan's operating table being vivisected, and dies as Denise screams. Later that night, Jerry Goldsmith, Bill Hudley, Mary Knowles, and Denise Willis are on the road in hopes of writing a book on offbeat roadside attractions. When the four meet Spaulding, who is also the owner of "The Museum of Monsters & Madmen", they learn of the local legend of Dr. Satan. As they take off in search of the tree from which Dr. Satan was hanged, they pick up a young free-spirited hitchhiker named Baby, who claims to live only a few miles away. Shortly after, a mysterious figure appears hidden in some overgrowth and shoots out their vehicle's tire with a shotgun. The group thinks it is just a blown out tire and so Baby takes Bill to her family's house to get a tow truck.
The movie's violence and sadism led to it not being able to shake an NC-17 rating, caused Universal to shelve it. In time, director Rob Zombie bought the rights, and went on his own harrowing adventure to find another distributor. MGM eventually picked up the film and scheduled it for release in October 2002, but it was dropped after Zombie reportedly joked about Universal having "no morals" for not distributing the film, and then, "Well, MGM picked it up. I guess they have no morals."

Charlie returns to save the Firefly family, but he is killed by Wydell.

His truck runs out of gas on the way, and he frightens a boy and assaults the boy's mother before stealing her car. Back at the motel, Otis rapes Roy's wife Gloria and demands Adam and Roy come with him on an errand. Some of these actors were a major part of the movie, present for the majority of its runtime. But whether they formed a huge part of the movie's machinery or acted as a single cog, each of these actors was part of something special.
There's an abundance of gore in this derivative horror movie, but little sense or wit. The Devil's Rejects and 3 from Hell have since rounded out the trilogy, so this might come as a surprise to many fans. But Zombie knew the troubles that accompany getting a movie funded, and as the filmmaker explained in a 2013 interview, "If someone's willing to put up that money and make your film happen, you really can't look that gift horse in the mouth."
